Advance Care Planning

Building an ecosystem between Woodlands Health and partners to raise awareness on Advance Care Planning.

About Advance Care Planning

Advance Care Planning (ACP) is the process of planning for one’s future health and personal care goals. It includes discussing one’s personal beliefs and goals for care with their loved ones and healthcare providers.

We are working on bringing ACP into the community, to enable more people to easy access to ACP outside of the hospital.
