About ​Frailty


Frailty is a condition common among​ older people and points to a high risk of poor health outcomes. 

Frail older persons usually suffer from unintentional weight loss, muscle loss and weakness that may lead to longer hospital stays or poor recovery when you are ill​.​ ​​​

CommFit is a frailty prevention and management programme targeting residents in Woodlands who are pre-frail to moderately frail.

It is led by our Community Health Team (CHT), with support from our Geriatric Service and Allied Health colleagues, which include physiotherapists, occupational therapists, dietitians, and pharmacists.

  • ​​Frailty assessment
  • Exercise advice
  • Nutrition education
  • Caregiver support
  • Medication review
  • Home environment assessment
  • Referral to other services​

CommFit will be provided at two of our Community Health Posts (CHPs):
