About Advance Care Planning​

Advance Care Planning (ACP) is the process of planning for one’s future health and personal care goals. It includes discussing one’s personal beliefs and goals for care with their loved ones and healthcare providers.

We are working on bringing ACP into the community, to enable more people to easy access to ACP outside of the hospital.​​

ACP Referral​ Programme​​​

​ACP Training​​

If you are keen to train your staff in ACP Advocacy or Facilitation (General ACP/Preferred Plan of Care), please email us at acp@wh.com.sg​.

ACP Advocacy training is suitable for anyone who is interested to know about ACP and wishes to advocate ACP to residents/clients/patients. This includes clinic assistants, centre managers, as well as outreach and engagement staff.

ACP Facilitation training is suitable for doctors, nurses, social workers, and allied health professionals.​
