About Woodlands Health@Home
Woodlands Health@Home (WH@Home) is a programme designed to assist patients with low to moderate acuity conditions, necessitating daily interactions as part of an early supported discharge from inpatient settings and to prevent admission from the Emergency Department, Specialist Clinic, and the Urgent Care Centre.
These patients will receive continuous support from a 24/7 Multi-Disciplinary Team (MDT) comprising community doctors, nurses, and allied health professionals.
Objectives
- Enable patients to receive quality care in the comfort of their homes as an alternative to hospitalisation
- Enable patients to shorten their stay in hospital, hence minimising the risk of complications associated with prolonged hospitalisation such as hospital-acquired infections.
