About us​​​

WH Post Acute and Rehabilit​ative Care (PARC) plays a crucial role in continuing patient care, providing rehabilitation and sub-acute clinical services.

Our rehabilitation physicians collaborate closely with acute hospital clinicians and surgeons, evaluating patients' rehabilitation needs and devising individualised treatment plans alongside acute care. Patients in need of PARC services are referred by their acute hospital physicians or surgeons, and admission requests are assessed to ensure optimised and appropriate care.

PARC's goal is to maximise each patient's rehabilitation potential. We strive to restore activities of daily living (ADLs), alleviate caregiver burden, and facilitate the integration of patients back into their homes, communities, or other step-down care facilities.


We provide the following services for:

1. Patients requiring rehabilitation after stabilisation of acute issues

  • ​Medical Care and Clinical Management
  • Assessment & Formulation of rehabilitation plans
  • Moderate intensity rehabilitation
  • Physiotherapy, Occupational Therapy, and Speech Therapy​
  • Rehabilitative Education & Caregiver Training
  • Discharge planning to empower patients’ independence in the community with the necessary support services where appropriate.

2. ​​​Patients requiring time-limited (2 – 4 weeks) medical care or stabilisation after acute phase of their illness or injury​

  • Maintenance of clinical management for diagnosed condition, including simple medication titration and continuation of medical treatment 
  • Clinical, Nursing and Allied Health Management, Assessment and Monitoring
  • Additional caregiver training
  • Continuation of care while undergoing further discharge planning

3. Rehabilitation Physician Liaison/Consult Service for patients​

  • ​Assessment and management of disability resulting from illness or injury, including:
    • ​Evaluation of medical, social, emotional, work and recreational aspects of function.​
    • Maximising independence and improving quality of life.
    • Prognosticate functional recovery.
  • Recommendation of best site of care in a range of settings including home, inpatient ward and community rehabilitation centres depending on a patient-centred, individualised treatment plan

  • Prescribe medications, exercises, adaptive devices and modifications. 

  • ​Offer selected interventions such as neuromuscular, musculoskeletal and joint injections.
