We provide subacute and rehabilitative care to support patients recovering from acute medical or surgical conditions, facilitating their discharge back to the community.

About Us​​

​

Our multidisciplinary team of medical, nursing and allied health professionals provide medical and nursing care as well as inpatient therapy to assist patients in their medical and functional recovery.

We aim to optimise medical conditions, improve function for activities of daily living (ADLs), provide caregiver training and support, and facilitate integration of patients back to their home environment or other step-down facilities.

We provide the following services​:

1. ​Rehabilitative care

  • Medical and Nursing Care

  • Assessment & formulation of rehabilitation plans

  • Mild to moderate intensity rehabilitation based on patient’s medical condition

  • Physiotherapy, Occupational Therapy, and Speech Therapy

  • Rehabilitative education & Caregiver training

  • Home environment assessment & Equipment prescription

  • Discharge planning to empower patients’ independence in the community with the necessary support services where appropriate.

2. Subacute care

  • Maintenance of clinical management for diagnosed condition, including simple medication titration and continuation of medical treatment

  • Medical and Nursing Care

  • Physiotherapy, Occupational Therapy, and Speech Therapy (where appropriate)

  • Caregiver training

  • Discharge planning to empower patients’ independence in the community with the necessary support services where appropriate

3. ​Hospital to Home Service

  • ​​Home visits and regular check-ins to ensure patients are supported in the post-discharge period

  • Empower patients to manage their health and well-being in their own homes and communities

  • Assesses care needs and connects patients to relevant community services

  • Aims to prevent unnecessary readmissions to the hospital
