About Us​

​Our team of rehabilitation physicians work closely with acute hospital clinicians and allied health professionals to assess each patient's rehabilitation needs and develop individualised treatment plans alongside acute care. 

Our goal is to help individuals regain function, improve independence in daily living, and manage any associated disabilities. With a collaborative and holistic approach, we support patients at every stage of their rehabilitation journey.​​

Our services​​
  1. Rehabilitation physician consultation service
    ​
    • Assessment and management of disability resulting from illness or injury, including:

    • Evaluation of medical, social, emotional, work and recreational aspects of function

    • Maximizing independence and improving quality of life

    • Prognosticate functional recovery

    • Recommendation of best site of rehabilitative care setting (eg. Home, inpatient ward, community hospital, outpatient community service providers)

    • Prescribe medications, exercises, adaptive devices and modifications

    • Provide selected interventions such as neuromuscular, musculoskeletal and joint injections

  2. Inpatient Services
    ​
    • Provide tertiary rehabilitation care in the acute hospital setting before eventual transition to other facilities as appropriate

    • Deliver comprehensive, physician-led multidisciplinary inpatient care specifically tailored for patients with complex rehabilitation needs (eg. Stroke, traumatic brain injury, spinal cord injury, polytrauma/amputee)

  3. Outpatient Services

    • Provide follow-up for post-acute rehabilitation, return to work, and selected interventions for complications post-injury/illness.
