About THRIVE Programme
THRIVE (Transforming Health & Resilience in Vulnerable Elders) offers specialised medical consultation and support for designated nursing home residents, delivered by a dedicated geriatric care team which includes the geriatric doctors and trained nurses.
The aim of the service is to reduce hospitalisation among frail elderly residents in NH.
The service also aims to:
- Review and consolidate SOC visits
- Review polypharmacy
- Review recurrent admitters
- Optimise medical conditions such as challenging behaviour
- Facilitate goals of care and ACP discussions
- Provide education and training for nursing home staff
Consultations are conducted through case discussions, followed by in-person consultations or video consultations if required.
