Transitional Care for Residential Home Residents
By working with Nursing and Welfare Homes in the north, we can tighten communications of residents’ admission, and co-develop post-discharge follow-up workflows. We started this initiative with Orange Valley Nursing Home, MWS Christalite Methodist Home and Metta Home, and hope to extend this to the rest of our partners.
