About Our Community Health Team


​​Our Community Health Team, which comprises a multi-disciplinary team of Community Nurses, Health Coaches and Care Coordinators, will engage and empower residents to take charge of their health and wellbeing. They will serve at each CHP on selected days and hours.​

Partners may use this form to refer residents to our Community Health Posts (CHP).​

Alternatively, residents may be directed to use this form​ to request for an appointment at ​​our CHPs​. ​

Community Health Posts

​​Our Community Health Posts (CHP) are touchpoints within the neighbourhood that will provide residents with easy access to health services near their homes.​

​

​​​Our Services

​We offer a range of services to support active and healthy living, and to help with post-hospital recovery and chronic disease management. We help residents with the following: ​

  • Plan health journey and goals 
  • Learn to manage chronic health conditions 
  • Participate in exercise and wellness programmes 
  • ​Connect with community care services​
