An Ecosystem of Care

You and your loved ones are central to how we care for our population. Our work focuses on keeping residents well and supported in the community. By ensuring a robust network of care support for patients, caregivers and ​families outside the hospital, we aim to help you and your loved ones be happier and healthier, for longer.

Collaborate​

Sustained success in the community for you living in the North is only possible with the strong support of partners in our eco-system working with us in building a healthy and thriving community together.

Here are a list of community programmes our residents can be a part to improve health and social engagement

 

Share a Pot®

Share a pot is a community-based project to improve the nutrition and fitness of community-dwelling seniors. Supported by volunteers, the project provides an opportunity for seniors to gather in their local community for a bowl of nutritious soup, exercise and regular monitoring to build brains, brawns, bones and bonds.

Community Partnership Programmes​

Partnerships with health and social care providers in the community are vital to ensure strong support for patients and residents beyond the hospital, and closer to home. Our Community Transformation Office and Community Nursing teams run the following programmes in collaboration with our partners.

  • Health talks
  • Cooking demonstrations
  • Physical fitness programmes 
  • Inter-generational activities by students from CHOPE​​​

Connect

Our community enga​gement efforts are anchored by CHOPE, which aims to build a Community of Hope as part of the Northern ecosystem of care, to better connect you and your loved ones to resources and opportunities in this thriving community.

 

CHOPE X Schools​

Fostering a greater sense of community and wellness in our Youth today will help ensure a brighter future for all. CHOPE X Schools is a collaborative effort to co-develop programmes and reimagining healthcare by engaging, empowering and mobilising youths as part of the supported self-managed ecosystem in the North.

We work closely with schools to facilitate experiential learning, mentorship and volunteer opportunities in local community and healthcare organisations.



 

CHOPE X Corporate​

Northern Singapore is home to a host of corporations who play an important role in building our economic resilience. WHC aims to partner members of the private sector to foster greater workplace wellness, volunteerism and contribution back to the community.

In addition, we also recognise the strengths and role local community organisations play. We actively reach out to neighbours in our community such as faith-based organisations to empower their volunteers and co-create meaningful programmes for the community they serve.
